PLEASE REMEMBER ABOUT SOME IMPORTANT FORMALITIES

INFORMATION FOR STUDENTS LEAVING NYSA

DURING THE EXAMINATION SESSION:
FIRST OF ALL REMEMBER THAT
YOU CAN DO THIS!
JUST KEEP CALM AND STUDY HARD :)

• Receive examination card from ICO (to be filled by your teachers)
• A check card need to be picked up from ICO:
  You need the signatures from the administration of the dormitory and library - if you are enrolled.
  The check card is a confirmation that you do not have any obligation to the University of Applied Science in Nysa, i.e. books that were not given back, financial liabilities (like dorm payment), dishes and cutlery that were not given back to administration of the dormitory etc.
• Fill in an evaluation questionnaire (please find in the attachment)
• Inform us about the date of your departure (it is important to calculate the costs of the dormitory for an incomplete calendar month and to prepare the final documentation for you)
  
  ONE WEEK BEFORE YOUR DEPARTURE:
  
• Give back filled check card and examination card to International Cooperation Office
• Send back filled evaluation questionnaire
• Take the deposit back from the dormitory
  
  THE LAST DAY OF STAY:
  
• Pick up the Transcript of Records from ICO
  Check if all the notes, subjects and signatures of the teachers for each course are correct
• Give back your student card to ICO
• Before leaving don't forget to give back keys at the administration of dorm

Please take into account the fact that we need check card, examination card, student card and evaluation questionnaire to prepare all documents for you before your departure.

• Receive examination card from ICO (to be filled by your teachers)
• Fill in an evaluation questionnaire (please find in the attachment)
• Give back filled examination card to International Cooperation Office
• Pick up the Transcript of Records from ICO
  Check if all the notes, subjects and signatures of the teachers for each course are correct

Please take into account the fact that we need examination card and evaluation questionnaire to prepare all documents for you.
